The Licensed Practical Nurse is responsible for identifying basic patient needs and care. The family or those significant to the family are included in the care of the patient, when appropriate. The nursing care provided is on the basis of actual needs and treatment and meets hospital and Division of Patient Care Services Standards of practice, policies and procedures and also reflects the philosophy of the department.JOB REQUIREMENTS:License/Certification:

Valid license to practice practical nursing in Louisiana. American Heart Association BLS-HCP. POSITION DUTIES: Faciliates and maintains a safe environment for patients and others: Performs y technical skills/procedures required by the area/department/assignment safely, effectively, efficiently and legally. Utilizes current knowledge, skills and abilities to deliver patient care, treatment or observation of the ill and for the mantenance of the health of others and the promotion of health care. Establishes and/or revises /patient care activities based on acuity of needs, resource availabilty, patient preference and other departmental needs under the direction of the registered nurse. Problem Recognition/Management: In response to data indicating potential/actual risk to individual's health, initiates action to correct, reduce or prevent the risk in collaboration with the registered nurse Gives a rationale for actions or decisions based on , successful experience, skills and knowledge or standards of practice/protocols. Contributes to the development of a plan of care for assigned patients in colloboration with the registered nurse Collaborates with interdisciplinary team members to identify actions to improve patient care. Seeks supervision, consultation, assistance when unable to perform effectively or safely independently. Uses interpersonal-communication strategies with individuals that are intended to achieve desireable/acceptable outcomes and a perception of satisfaction by those involved. Uses internal and/or external resources to resolve and/or prevent problems that cannot be managed independently. Initiates and/or maintains interventions to assure continuity of safe/effective care of patients including medication management and patient instruction. Complies with established hospital/departmental/personnel policies. Documents consistent with established guidelines.
