Job Description

The Nurse Navigator for Collegiate Programs (NNCP), utilizing innovative and flexible solutions, program planning and best practices with an emphasis on standardization and high reliability, is responsible for establishing an environment that fosters teamwork, role competence and proficiency in the function of the Collegiate Programs throughout Ambulatory.

The Nurse Navigator for Collegiate Programs reports directly to the Associate Chief Nursing Officer and works closely with ambulatory leadership and a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care in the collegiate clinical settings throughout ambulatory.

The Nurse Navigator for Collegiate Programs collaborates with the interdisciplinary care team in developing, coordinating, overseeing and administering care to our collegiate affiliates in accordance with contractual agreements.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

  • The NNCP is a leader within nursing providing direction and priority setting in regards to patient care.
  • The NNCP acts as a resource for all staff in regards to education, planning, relocating and development of clinical programs pertaining to the collegiate programs.
  • The NNCP will be responsible for overseeing all clinical support staffing in the collegiate arena onsite and offsite.
  • The NNCP, in conjunction with ambulatory clinical and administrative leadership, ACNO, and collegiate partners coordinates any transition of patient care from one college based center to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.
  • The NNCP identifies high risk patients and works with the appropriate services, collegiate partners and interdisciplinary team members to achieve the best possible patient care and outcomes.The NNCP educates the patient and /or family regarding the patient’s clinical condition, treatment, and role in their healthcare decisions.
  • The NNCP develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research staff and collegiate partners thus producing high performing, patient focused teamwork.

Qualifications

A. JOB KNOWLEDGE AND SKILLS:

  • Functions autonomously at top of licensure
  • Demonstrates ability to lead and direct staff
  • Serves as a visible leader and role model in the development/attainment of hospital and unit service line mission and goals

  • Demonstrates expert knowledge and skill in patient care within ambulatory setting
  • Experienced in patient-focused, family-centered care
  • Inspires and achieves outcomes in staff learning through formal/informal teaching methods
  • Provides consultation in the delivery of care that inspires excellence in knowledge, skill, and leadership at the clinic level
  • Demonstrates quality in clinical performance as evidenced by creativity in problem solving, risk taking and interdisciplinary communications
  • Ensures evidence-based, best practices and standards of care are being practiced clinically
  • Possesses expertise in building high functioning teams and processes; promotes teamwork and interdisciplinary collaboration amongst entire healthcare team
  • Works to achieve outcomes in collaboration with members of the leadership team for all quality, safety, patient experience metrics and regulatory requirements
  • Embraces the concept of change and leads staff through the change process as evidenced by adoption of new behaviors and/or processes
  • Exhibits excellent communication skills
  • Flexibility in work schedule to accommodate clinical site needs

B. EDUCATION

  • BSN required
  • Advanced nursing education through the pursuit of masters or higher degree preferred

C. LICENSES, ECT

  • Massachusetts Registered Nurse License required
  • BLS required
  • Specialty Certifications highly preferred

  • Demonstrated ability to develop and engage in patient communications that are age appropriate

V. WORKING CONDITIONS/PHYSICAL DEMANDS:

  • Ability to oversee multiple collegiate sites on campus and off campus; frequent travel between sites as needed
  • Prolonged, extensive, or considerable standing/walking
  • Lifts, positions, pushes and/or transfer patients and equipment
  • Considerable reaching, stooping, bending, kneeling, crouching
  • Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc
  • Regularly exposed to the risk of blood borne diseases and other transmissible infections
  • Contact with patients under wide variety of circumstances
  • Subject to varying and unpredictable situations
  • Handle emergency and crisis situations
  • Subjected to irregular hours
  • May have contact with hazardous materials.
