If BSN enrollment in health related Master's program within 2 yearsTwo to three years inpatient care management; experience working in an acute care environment. Knowledge of discharge planning; community based resourcesAbility to interact with stakeholders ( providers; payors; patients ; families; vendors; post acute providers. Knowledge of CMS CoP for discharge planning. Ability to problem solve, utilize critical thinking effectively.Preferred Qualifications: Master's degree in health related fieldExperience precepting new orientees; mentoring and coachingLicensure and Certifications: Active Pennsylvania RN license if applicableRN license or Master of Social work;ACMA Certification in Case Management or CCM.ACMA certification within two years being in position.Influenza vaccination is required at most LVHN facilities for all colleagues with patient contact unless medically contraindicated or due to a bona fide religious belief or practice, and is strongly encouraged for all colleagues.
