Ann Pearl Rehabilitation & Healthcare in Kaneohe, Oahu is looking for a Registered Nurse (RN-Charge Nurse) to help us with providing professional nursing care to residents following established policies and procedures of the facility. Assists in management and supervision of the nursing unit. Charts and makes observations. Oversees and directs the C.N.A.s in their day-to day assignments. Answers questions from residents, responsible parties and other personnel. Oversees use of supplies and equipment.

Shift: Non-Exempt Full-time & Casual - Evening and Night Shift variable

Full-time (40 hours or more a week), Part-time (20 hours or more a week), Casual (less than 20 hours a week)

Day Shift (6 AM - 2:30 PM), Evening (2 PM - 10:30 PM), Night (10 PM - 6:30 AM)

Duties include but are not limited:

Administers medications as ordered by the physician and according to policy and procedures.

Takes telephone orders from physicians and transcribes orders to new medication administration record, the pre-printed physician order sheet, resident chart, treatment/care plans as appropriate.

Maintains appropriate inventory of resident medications and medical stock supplies.

Orders required medications/treatments from the pharmacy / other supplier.

Administers treatments as ordered by the physician and according to policy and procedures.

Conducts accurate physical assessments of residents, determines their needs and implements appropriate interventions to meet those needs.

Performs assessments on admission, transfer and/or discharges.

Appraises the family, physician and others of changes/incidents.

Documents changes in the residents' status in the progress notes and alert charting as necessary.

Documents a monthly summary on each resident as assigned, utilizing the care plan as a tool to assess resident status and as a means to update a resident's plan of care. Complete resident monthly assessments as assigned by MDS nurse.

Communicates specifics of care for each resident to the C.N.A. Oversees care delivery by CNAs.

Assists in preparing residents for outpatient appointments and communicates information to family members.

Follows-up on nutrition and hydration status of residents.

Initiates monitoring and documentation for residents that require monitoring in specified areas of care.

Ensures infection control processes and procedures are followed by all staff on the unit. Completes infection control surveillance forms accurately and in a timely manner.

Orients and supervises new staff as assigned.

Maintains the Daily Census Report per facility protocols.

Collaborates with physicians, consultants, agencies to improve quality of services and to resolve identified problems.

Performs minor housekeeping duties as needed. Maintains safety protocols and requires assigned staff to do the same.

Requirements:

Graduate of an accredited school of nursing.

Certification Current license as s Registered Nurse in the State of Hawaii required.

Experience in geriatrics and current nursing standards of practice preferred.

Knowledge of medical terminology and abbreviation required.

First Aid and CPR preferred. (can be done in-house).

Must have current medical/health screening including 2 step TB clearance (can be done in-house).
