The Villas in Honolulu, Oahu is looking for a Social Worker to manage the delivery of support services to residents including the identification and assessment of psychosocial, emotional, environmental, spiritual, discharge and support needs; provision and coordination of counseling services; arrangement for proper admissions, discharges, and transfers; coordination of abuse prevention program and abuse investigations. Our Social Worker strives to provide a smooth transition from an acute care environment, with special consideration on developing a plan of care which will support a safe and appropriate discharge. They support our guests, and their families, from the time of admission, throughout their stay, and finally as they transition back into the community. Our Social Workers also lead abuse prevention programs as an integral part of an interdisciplinary team.

Shift: Exempt Full-Time, Weekends Required, Regular Business Hours (8 AM - 4:30 PM)

Duties include but are not limited:

Identifies and assesses upon admission the resident's psychosocial, emotional, environmental, spiritual, and support needs in order to assist in developing a plan of care designed to meet that patients/resident individual needs. Involves the resident and/or family in the plan of care. Reassesses as needed.

Provides psychosocial counseling and crisis intervention to facilitate effective coping/problem solving strategies in relation to illness/disability, death, and/or emotional and relationship problems.

Manages process by assuring that all residents have a valid and accurate in a timely manner.

Completes or ensures the completion of admission agreement with new residents and their families.

Completes or ensures completion of the process for Social Services to include: initial assessments, state/federal timed assessments, and care plans.

Documents services and resident care activities according to established policy and procedures to assure timely and accurate completion of records.

Involves resident and family in the discharge planning process.

Maintains abuse protocol and investigates all allegations of abuse. Informs the Administrator of the report of abuse as soon as reported.

Coordinates and attends meetings.

Receives, investigates grievances, coordinates and attends family and resident conferences for grievances as necessary.

Recommends for selection, trains, orients and assigns department staff and managers.

Identifies training needs of department and conducts in-service training as appropriate or obtains appropriate resource to provide training.

Develops, revises and maintains all current forms and department policies and procedures in accordance with facility procedures.

Processes invoices for purchases in department is accountable for maintaining department performance within established budget.

Requirements:

Current Hawaii drivers licensed required.

Bachelor's or Master's degree in Social Work preferred with Hawaii State License.

Licensure/Hawaii State Social Work license or eligible preferred.

Minimum of 2 years of experience in long term care, a healthcare setting or work with the elderly preferred. Knowledge of medical terminology preferred.

First Aid and CPR preferred. (can be done in-house).

Must have current medical/health screening including 2 step TB clearance (can be done in-house).
