The responsibility of this position includes the day to day restorative based intervention with designated individuals in the skilled nursing/rehab environment under the direct supervision of the designated restorative nurse for all restorative related tasks. Also this position will collaborate and report directly with the TCN in accordance with the policies and procedures as established by TMC, the facility, payer sources, state and federal practices. Restorative aides will strive to provide the highest quality of resident care at all times while focusing on decreasing risk of decline.

Duties and Responsibilities:

 Carryout designated restorative programs with identified residents including but not limited to:

o Range of Motion

o Splint or Brace Assistance (Amputation/Prosthetic Care)

o Functional Mobility (transfers, bed mobility and ambulation)

o Positioning

o Activities of Daily Living (Dressing, grooming, bathing and toileting)

o Eating and/or swallowing (Restorative/Assisted Dining/Free Water)

o Other; may include: communication, breath support and wellness

 Document resident attendance and participation in designated programs.

 Provide feedback to rehab director, restorative nursing staff and interdisciplinary team on current status, improvements or declines of residents on program.

 Use universal precautions during interactions where bodily fluid contact or infectious/hazardous materials are involved.

 Attend all facility or TMC trainings or meetings as requested by rehab director.

 Identify and pursues own professional growth and development.

 Schedule and prioritize own workload with collaboration from Rehab Director.

 Maintain organized and clean treatment area, equipment, and supply inventory

 Follow HIPPA guidelines in relation to resident privacy.

 Other duties as assigned.

Qualifications:

 Graduate of a High School or equivalent program required.

 Certified Nursing Assistant experience preferred.

 Experience with Geriatric wellness programs encouraged.

 Must demonstrate the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others without risk to self, co-workers, caregivers, or patient; lift, turn, move position, and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

 Must demonstrate the ability to perform a minimum of minimal assist transfer.” Minimal assist transfers are defined for purposes of this document as the ability to lift and control 50 pounds.
