RN/LPN for DD Waiver Agency

Maxcare, Inc. is currently seeking an RN/LPN to support individuals who receive Developmentally Disabled Waiver Services. Our nurse will work with Individuals, Family Members, Guardians, Program Directors, Case Managers and other Providers to determine the Individual’s health care needs.

Duties Include: face-to-face visits are currently being conducted over the phone or Zoom due to Covid-19)

Input Individual’s information into Therap

Individual Annual Nurse Assessments

Provide trainings to Direct Support Staff on Individual Health Care Plans, Medical Emergency Plans and CARMPs

Conduct face to face visits per assessment acuity levels

Work with Program Managers, Service Coordinators and be available for calls and consultation as needed

Review Health Care Plans and Medical Emergency Response Plans every six months

Semi-Annual and Annual Nursing Reports

Create Health Care Plans and Medical Emergency Response Plans as needed

Attend Annual Individual Service Plan meetings and Interdisciplinary Team meetings

Maintain current CPR Certification, AWMD, ANE and other required State trainings

Computer skills (Word, Excel, e-mail)

Qualifications:

Valid Driver’s License

Reliable Vehicle

Current Car insurance and Registration

Experience working with individuals with intellectual and developmental disabilities preferred

No DWI/DUI in the past five years

Pass observed drug test

Able to pass State and Federal background check

CPR/1st Aid Certification (Preferred)

Current clinical license

Pay:

RN $36.00-$42.00 DOE

LPN $28.00-$32.00 DOE

Schedule:

Monday - Thursday 8:30am – 3:30pm or Monday to Friday 9:00am – 2:30pm
