Express is recruiting for a Case Manager to join a great non-profit in Arcata CA! The pay is $19.55/hr and this is a full-time position with flexible hours!

Duties and Responsibilities:

- Interview and select applicants for the program;

- Conduct client meetings and life skills education, if appropriate;

- Supervise and support clients in keeping living space safe and clean;

- Support clients in applying for mainstream benefits (veterans benefits, SSI);

- Provide clients with support in obtaining mental, dental, preventive, and primary care;

- Provide clients with support in obtaining substance abuse treatment services;

- Provide nutrition counseling, teach clients to shop using CalFresh, prepare food, and serve meals;

- Provides limited transportation to clients to obtain the above services and benefits;

- Encourage client independence;

- Report program outcomes accurately (electronic HMIS and written reports);

- Maintain accurate client case notes and other documentation as required, and provide crisis management;

- Assist with creating goals and encouraging client independence;

- Ensure clients are complying with program guidelines and procedures;

- Delivers services with safety, respect, and effectiveness;

- Assess client needs and offer assistance;

- Monitors client behavior;

- Follows all policies and procedures of the organization;

- Complete all agency logs and incident reports in a timely manner; and

- Provide an immediate and professional response to any security /safety emergency or disturbance within or immediately outside the facility

Minimum Qualifications:

- Bachelor’s Degree preferred

- 2 years demonstrated case management experience with the homeless preferred or low-income, at-risk clients (with a history of substance abuse, family violence, or mental illness for a social service agency) OR any equivalent combination of education and experience which has provided the applicant with the knowledge, skills, and abilities. This should include knowledge of trauma-informed care and harm reduction.

Computer Skills

To perform this job successfully, an individual should be competent in Microsoft Office applications, Google Suite, and the ability to learn and use computer software as necessary. Ability to use basic office equipment, including a cellular phone.

Communications Skills

- Ability to read, analyze, and apply common sense understanding and to carry out instructions in written or oral form.

- Ability to communicate with all levels of personnel, clients, and supporters.

- Ability to establish trust with clients and community partners.
