Qualifications
Driver's License
Photo Copy of Social Security Card
Local Background Check with No Criminal History
Certification (RN, LPN, CNA, PCA)
Copy of Certificate(s)
Print Out of Resume (Must be brought to interview)
Letter of Recommendation from a previous employer
Current TB Skin Test
Current BLS Certification Card
Current First Aid Certification Card
Pre-employment Physical with NO Work Restriction
Auto Insurance
Bank slip w/ account and routing numbers or voided check
Must Have Gchexs Finger Print or Pay Upon Interview.
