Job Description
The RN Full-Time Weekend team member will coordinate and provide care for an assigned and on-call group of Home Care and Hospice clients using the nursing process and assuring the nursing and other health care specialty needs of the client are met. The weekend shift will begin at 5:00 pm Friday and continue throughMonday at 8:00 am.The RN will maintainknowledge of Home Care and Hospice principles and guidelines. Ensures strong customer satisfaction with all services. Manages overall a case of population group of patients. Home Health and Hospice experience preferred.
Conducts initial evaluation visits assessing the client’s clinical, subjective and objective data, capturing a strong clear picture of patient at admission, and develop POC to meet patient’s specific needs. Conducts accurate OASIS assessments and meets regulatory timelines. Reassess the client’s status and revises plan of care as goals are achieved resolving them. Submits completed assessments within time frames determined by agency policy and/or regulatory requirements.
Qualifications
Registered Nurse (RN) with current Oklahoma nursing license
