RN or LPN - On Call Evening Home Health Full Time

Job Description

Responsibilities include working on call evening shifts, following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Organizes and directs/coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

The position would have regularly scheduled visits as well as on call visits.

Shift is evenings Monday through Friday 5pm to 8amand rotating Saturday and Sunday

Qualifications

1. Currently RN or LPN in state of Oklahoma.

2. Current CPR.

3. Experience at least 2 years in home health preferred.
