CLIENT also works to support caregivers through service, advocacy, and education.The Social Adult Day Care (SADC) Ombuds Office (“Office”) is charged with accepting & monitoring NYC social adult day care program registrations; and responding to SADC-related complaints or inquiries. The Office is seeking bids for a highly capable SADC Ombuds Office Projects Consultant with strong project management and analytical skills to support the Office through analysis, development, implementation, coordination and monitoring of SADC registrations, investigations, corrective action plans and notice of violations.

SCOPE OF WORK
  • The Consultant will work under the supervision of the Director of the SADC Ombuds Office and the essential duties and tasks will include:
  • Manage special projects based on Office needs such as conducting complaint investigations, and registration site visits throughout NYC.
  • Maintain electronic complaint tracking system to support the coordination and management of on-going complaint and registration projects.
  • Develop reports and perform analysis utilizing various data sets from the SADC Ombuds Office Complaint Tracking System and registration excel reports to inform management on quality assurance and policy issues leading to solutions to SADC complaints or registrations.
  • File documents in the appropriate electronic and physical files.
  • Make recommendations to the Office on project planning, procedures, and effective/ efficient use of resources toward meeting project targets and timelines.
  • Complete required documentation and reports of investigations and all other records, in a timely manner.
  • Collaborate with the Office, DFTA’s Office of Information Technology (OIT) and NYS Oversights (Office of Medicaid Inspector General, Office for the Aging and Department of Health) when developing and rolling out new trainings for SADC providers and Managed Long Term Care (MLTC) plans.
Deliverables:
  • Capture all Office projects as it relates to open/closed investigations; active/inactive SADC registrations; and provide monthly updates on status of each
  • Enter complaints into the SADC Ombuds Complaint Tracking System,
  • Coordinate with SADC providers and MLTC plans on Corrective Action Plans (CAP), when needed; and provide updates on a weekly basis.
  • Develop registration and complaint reports and provide weekly metrics for program
  • Collaborate with Office of General Counsel on all issuance of notice of violations, and/ or civil penalties, and testify at appeals/ hearings, when
  • Record all SADC Ombuds Office activities
  • Conduct and track monthly outreach activities to SADCs to ensure that SADC registration information is up to date.

  • Strong analytical

  • Demonstrated ability to manage multiple complex projects to completion, preferably in a government or community-based setting.
  • Excellent organizational skills with ability to take initiative, problem solve, prioritize duties and work independently in a fast paced and team environment.
  • Excellent oral and written communication skills and capacity to work both independently and as part of a team
  • Proficient in Microsoft Excel, Word, Access, PowerPoint, and ability to use other technology/
