Adult Day Health Care Center seeking full therapy team:

Physical Therapist

1. Evaluates all participants’ physical condition and functional status. Establishes a realistic goal and sets up a treatment plan to administer physical therapy techniques. Consults with participant, family and caregiver to determine wants, needs and goals for day center attendance and physical therapy interventions. Completes signed and dated quarterly progress notes, and six-month reassessments on all participants receiving Physical Therapy. Reevaluates patient for any changes is health or functional status.

2. Provides physical therapy treatment. Instructs and demonstrates to family and others the physical therapy and rehabilitation procedures indicated.

3. Provides general instructions in principles of physical therapy to staff members as needed.

4. In coordination with the center R.N., supervises maintenance aides by instruction, observation and open discussion.

5. Records on participant's charts as daily. Records initial and final evaluations, makes progress notes, and schedules participants for further treatment.

6. Works in close cooperation with staff and participants as a member of the Multidisciplinary Team.

7. Provides a maintenance-training program to center staff and periodic evaluation of those receiving training.

8. Meets the administrative requirements of the position such as attending meetings and keeping statistical records.

9. Participates in the discharge plan and initials the discharge summary.

Occupational Therapist

1. Evaluates all participant's physical condition and functional status. Establishes realistic goals and sets up treatment plans to administer occupational therapy techniques. Consults with participant, family and caregiver to determine wants, needs and goals for day center attendance and physical therapy interventions. Completes signed and dated quarterly progress notes and reassessments at least every six months. Reevaluates patient for any changes is health or functional status.

2. Provides occupational therapy treatment. Instructs and demonstrates to family and others the occupational therapy and rehabilitation procedures indicated.

3. Provides a maintenance-training program to center staff and periodic evaluation of those receiving training.

4. In coordination with the center R.N., supervises maintenance aides by instruction, observation and open discussion.

5. Records on participant's charts daily. Records initial and final evaluations, makes progress notes, and schedules participants for further treatment. Completes quarterly progress notes, and six-month reassessments on participants for all participants

Speech Therapist

1. Evaluates the participant's physical condition and functional status. Establishes a realistic goal and sets up a treatment plan to administer speech therapy techniques. Re-evaluates participants on a regular basis and upon change of status or condition.

2. Provides speech therapy treatment. Completes swallowing evaluations as indicated by the needs of the participant. Instructs and demonstrates to family and others the speech therapy and rehabilitation procedures indicated.

3. Completes signed and dated quarterly progress notes, and six month reassessments on participants receiving Speech Therapy.

4. Provides general instructions in principles of speech therapy to staff members as needed to work with the participants.

5. Records on participant's charts as services are provided. Records initial and final evaluations, makes progress notes, and schedules participants for further treatment. Records all treatments on the same day they are provided.

6. Works in close cooperation with staff and participants as a member of the Multidisciplinary Team.

7. Meets the administrative requirements of the position such as attending meetings and keeping statistical records.

8. Participates in the discharge plan and initials the discharge summary.
