Job Description
Job Responsibilities- Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
- Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.
- Obtains and documents prior authorization for services from insurance providers.
- Ability to demonstrate compassion and courtesy when dealing with difficult situations.
- Capable of working under pressure and time constraints including overtime to complete daily assessments.
- Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.
- Must possess the ability to use and apply clinical knowledge to the referral process.
Qualifications
Required Skills- Minimum of one year of experience in intake coordinating.
- Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
- Excellent problem-solving, observation, written and verbal communication skills.
- Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.
- Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations.
- Effective interpersonal communication skills.
- Fluent knowledge of English, both verbal and written.
