Job Description

Provides comprehensive navigation services to those who are experiencing homelessness or at-risk of becoming homeless in the City of Aurora. The position works as part of a larger interagency collaboration of [emailprotected], the Veterans Administration Medical Center as well as internal depatmentsto coordinate outreach services, housing placement and navigation of services for those eligible and admitted into the [emailprotected] program. The ultimate goal is to assist veterans in finding housing and supporting them in achieving housing stability.

1. Works collaboratively with service providers serving families experiencing homelessness in the Aurora metro area to provide optimum coordination, communication and continuity of services to those who are experiencing homelessness or who are at-risk of becoming homeless.

2. Advocates for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of persons who are experiencing homelessness.

3. Builds relationships with clients with the ultimate goal of achieving housing stability .

4. Treats each person contacted with dignity and respect by using a Trauma Informed approach.

5. Maintains client files with up-to-date and accurate information (e.g., intake documents, assessments, income verifications, employment verifications, rent calculations, other funding specific paperwork, recertification documents, compliance letters, case notes, etc.).

6. Develops individualized action plans for veterans.

7. Obtains all needed data on the veteran's progress and overall program outcomes.

8. Works as a member of the [emailprotected] collaborative by communicating effectively, generating reports as required, participating in or convening case conferencing meetings as needed.

9Transports veteransduring housing search process and when accessing other needed services or resources. Requires the use of personal vehicle covered by required insurance

10.Works to recruit owners and property managers who are willing to serve veterans. Collaborates with the [emailprotected] Landlord Recruiter

11. Assist veterans in identifying housing, leasing-up and retaining housing.

12. Mediates differences between owners/property managers and veterans as needed

13. Plans and organizes special events for veterans as needed.

Other duties as assigned.

Qualifications

  • Masters of Social Work (MSW) is required
  • HonorablyDischarged veteran Preferred
  • Two years experience in navigation/outreach/case management with families experiencing homelessness. Or two years in a related direct human service field working with families who are underserved.
  • Experience, competence and sensitivity in working with homeless individuals, especially those who have difficulty engaging in treatment, suffer from mental illness or substance addiction or are otherwise impaired.
  • Knowledge of the Veteran Administration Medical Center system.
  • Knowledge of community resources in Aurora metro area.
  • Skill in operating office equipment, such as computers, software (e.g., Microsoft Word, Excel and PowerPoint; e-mail) and telephones.
  • Skill in negotiating and mediating, particularly in sensitive situations.
  • Able to communicate effectively with diverse population.
  • Able to make sound decisions, using available information while maintaining appropriate confidentiality.
  • Able to function as a member of an interdisciplinary team and work with collaborating agencies.
  • Able to manage tasks and activities in an environment that sometimes includes interpersonal conflict and chaos.
  • Available to work flexible hours which may include combination of early mornings, late evenings and some weekends.
  • Able to pass automobile insurance carrier’s motor vehicle record investigation and CBI/Child Abuse Registry checks.

Up to 12months of on-the-job training to perform the job effectively

Conditions of employment:

Valid Colorado Driver License and Insurance

Transporting clients in your private vehicle is part of your job responsibilities. Your private insurance will be the primary insurance coverage. While the State of Colorado requires limits of $25,000 per person for bodily injury, $50,000 per accident for bodily injury. AHA strongly recommends carrying at least $100,000 per person and $300,000 per occurrence.
