The Weld County Department of Human Services Area Agency on Aging is searching for a Options for Long Term Care Case Manager to perform initial screening and functional assessment of referred clients to the Options for Long Term Care programs. Develop care plan for eligible clients and determine appropriate OLTC program for services. Provide ongoing case management services to residents of Weld County in order to meet their long term care needs in the least restrictive and least costly setting.

$22.05 - $27.93 per hour plus benefits (see benefits tab on the job posting).

As an applicant for this position, you should be aware of Senate Bill 19-085, the Equal Pay for Equal Work Act. The act requires employers to announce the pay range for job openings. This range encompasses the nine pay steps available should you receive the position and demonstrate satisfactory performance over time.

Understand and manage all the components of the Home and Community Based Services Waiver Programs (HCBS), Programs of All-Inclusive Care for the Elderly (PACE), the Nursing Facility placement process and State only long term care programs administered by the Single Entry Point/Options for Long Term Care (SEP/OLTC). Understand and manage the components of the Private Case Management functions of the SEP.

Complete initial screening of referred clients to determine potential eligibility for long term care programs and/or referral to appropriate community services. Complete comprehensive functional assessments according to program regulations and mandatory time frames to determine eligibility for Medicaid long term care programs. Participate in the appeal hearing process for denials of eligibility or services.

Develop and implement a written person centered plan of care which identifies specific services to meet each client's care needs as well as individual goals for those services and adjust as needed. Authorize all long term home health services as well as non-skilled waiver program services ensuring appropriateness and necessity of services according to program regulations while conforming to State cost containment guidelines.

Apply case management principles to manage a diverse caseload of eligible clients as required by regulations. Maintain ongoing contact with clients, via home visits and phone calls. Maintain communication with service providers as required by regulations to provide support and ensure services are implemented as planned and remain appropriate and necessary. Adjust services as necessary to continue to meet client needs.

Provide support and advocate for clients, families and providers during conflict situations and assist with problem solving to identify the best solutions to meet clients' long term care needs including potential, future needs that may arise.

Accurately establish and maintain client physical and digital files in accordance to SEP contract and program regulations including maintaining all standards related to the Benefits Utilization System (BUS). Maintain all standards related to client confidentiality and HIPAA regulations.

MINIMUM QUALIFICATIONS

Education:

Successful completion of a Bachelors Degree in a Human Services or related field, such as Gerontology, Psychology, Rehabilitation, or Sociology. Official transcripts must be provided.

Required Work Experience:

One year full-time work experience in a human services environment.

Preferred Work Experience:

Experience working with a diverse population including disabled individuals of all ages, aging populations and individuals with behavioral mental health needs.

Experience working with or knowledge of community resources and other community based services for the aging or disabled population.

Experience performing needs assessments and/or care planning for individuals.

Experience determining eligibility for program or supportive services.

Substitution of Work Experience:

Successful completion of an approved internship in human services may substitute for the work experience on a month by month or year by year basis.

Required Skills, Knowledge and Abilities:

Must have a valid Colorado Driver's License and Liability Insurance. Must provide own transportation and be willing to travel throughout Weld County.

Must pass Human Services Background Checks.

Preferred Skill:

Ability to keyboard at 35 words per minute.

Testing may be required for applicants who are selected to move forward in the hiring process.
