About the role:

This rewarding and varied role would suit someone who is highly motivated, has excellent customer engagement skills and enjoys working autonomously.

The main focus of the role is to manage a portfolio of community-based customers who have been allocated home-care packages. The main responsibilities include identifying and mitigating risks, developing tailored care plans, managing budgets and coordinating care services to enhance our customers quality of life and maintain their independence. You will be inspired and motivated by making a positive difference to the families and achieving ultimate customer satisfaction.

Your strong account management and networking skills will enable you to build rewarding external relationships within the relevant communities. Your week will be spent mostly out on the road around Port Macquarie and surrounding areas visiting customers in their homes, while the rest of your time will be spent liaising closely with our internal teams to ensure the smooth delivery of care services.

This is a flexible and autonomous role where you’ll have the ability to manage your own schedule.

About you:

  • Previouscase management experience and/or knowledge of the Community/Aged Care system and home-care packages.
  • Strong leadership, coordination and organisation skills.
  • Exceptional customer service, negotiation and influencing skills.
  • Experience in a results driven environment.
  • Excellent ability to connect with multiple stakeholders and think commercially.
  • Experience using a customer database or online CRM tool.
  • A current valid driver's licence and reliable vehicle.
  • A clinical background (RN, allied health, social work, dietician, mental health worker etc) would be great but not essential.
