Position Title: Therapeutic Foster Care Case Manager/Safe Writer

FLSA Status: Full Time Non Exempt

Required Travel: Local Moderate to High 30-90%

Salary Range: Hiring Range 18.11- 19.74 per Hour DOQ

Position Summary

The Case Manager is responsible for the case coordination of foster clients and foster parents.

Essential Duties and Responsibilities

1. Establish, build, and maintain rapport with children in care, foster homes, kin, and other members of the multidisciplinary team in order that optimal information gathering and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of all involved.

2. Monitor and ensure safety of clients and assist/advocate for them to reach their goals and full potential.

3. Coordinate case with providers and support goals of Family Service Plan.

4. Maintain a clinical record for each client/family per agency standards and adhere to volume 7 regulations ensuring files are always audit ready.

5. Responsible for case management, including the coordination of services, case related treatment team coordination, medical/mental health, academic and all other appointments as deemed necessary for the children placed by the agency.

6. Participate in mandatory rotation for 24 hour “on call” to support our clients and families.

7. Participate in group/individual supervision sessions.

8. Represent Ariel Clinical Services by acting as a liaison to local human service agencies and state level agencies, as required/assigned.

9. Monitor the TFC Homes to ensure compliance with rules and regulations and assist in training through regular contact and a minimum of monthly home visits.

10. Conduct full/update Structured Analysis Family Evaluations (SAFEs), as qualified.

11. Complete monthly reports and participate in peer review of assigned reports.

12. Participate in ongoing program evaluation to ensure program effectiveness.

Required Knowledge, Skills and Abilities

1. The ability to effectively communicate verbally and in written format in both individual and group settings.

2. Ability to be flexible, and able to change direction rapidly.

3. Ability to maintain strict confidentiality with all records and information.

4. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.

5. Basic computer knowledge.

6. Ability to organize and present case material in a straight forward and concise written manner.

7. Knowledge of human development with regard to major milestones of development and normal development parameters.

8. Knowledge of family dynamics with regard to communication, interactions, and general functioning as well as parenting skills and intervention techniques.

9. Ability to provide reliable, insured transportation for self and others through the community served.

10. Ability to demonstrate consistent and reliable attendance as scheduled and requested.

Education Requirements

Required:

Bachelor’s degree in social or behavioral sciences.

OR

Bachelor’s degree in a non-social or behavior science, and,

Two years experience supervised by an individual with a MSW or other Masters Degree in the social or behavioral sciences.

Preferred:

3 years or more experience in child and/or family services.

Structured Analysis Family Evaluation (SAFE) certification at hire with prior related experience.

1 year I/DD or mental health experience.

Certifications and Licenses

Valid Regular Drivers License, with acceptable driving record.
