Stay At Home Healthcare Services, located on the island of Hawaii, offers home health care treatments by health professionals that are as effective as those provided in a hospital or skilled nursing facility.The Position:

Stay at Home Healthcare Services is looking for a caring and passionate Social Worker to manage the delivery of support services to clients including the identification and assessment of psychosocial, emotional, environmental, spiritual, discharge and support needs. They support our clients and their families as they transition back into the community.
