Supv, Clinical Pharmacy/MTM (REMOTE)Irving, TexasJob DescriptionJob SummaryMolina Pharmacy Services/Management staff work to ensure that Molina members have access to all medically necessary prescription drugs and those drugs are used in a cost-effective, safe manner. These jobs are responsible for creating, operating, and monitoring Molina Health Plan's pharmacy benefit programs in accordance with all federal and state laws. Jobs in this family include those involved in formulary management (such as, reviewing prior authorization requirements, reviewing drug/provider utilization patterns and pharmacy costs management), clinical pharmacy services (such as, therapeutic drug monitoring, drug regimen review, patient education, and medical staff interaction), and oversight (establishing and measuring performance metrics with regard to patient outcomes, medications safety and medication use policies).Knowledge/Skills/AbilitiesThe Supervisor, Clinical Pharmacy/Medication Therapy Management (MTM) supervises Pharmacists, Pharmacist Interns, Technicians and other staff of a clinical pharmacy outreach service that educates patients and their health care team to optimize medication related healthcare outcomes, ensure patient safety, recommend cost effective medication strategies, coordinate care with the member's care team and comply with CMS guidelines. These staff may also be involved in providing utilization management/prior authorization services as needed. Supervises the unit's daily operations to ensure prompt and appropriate outreach and coordination of services, including MTM services for Medicare and Medicaid eligible members as well as Medication Adherence Programs for STAR, HEDIS and behavioral health medications. Ensures that adequate staffing coverage is present at all times of operation. Acts as liaison to internal and external customers to ensure prompt resolution of identified issues. Ensures compliance with all CMS guidelines and reporting requirements for the MTM Program; fulfills all required process and outcome measurements and represents the department in the CMS annual data validation audit; ensures compliance withMolina policies and procedures. Identifies and coordinates the notification of members who qualify for MTM. Assists with clinical research projects to improve the MTM Program. Assists in coordination of care for pharmacy related issues between members, providers and other members of the member's interdisciplinary care team, including referrals to state plan health care services. Analyzes and reviews medication lists for potential patient safety problems, including potential drug interactions, suboptimal medication regimens, etc. Provides education to members and their health care providers about the members' medications and appropriate interventions. Conducts comprehensive and targeted medication reviews for eligible members quarterly and provides new reconciled personal medication lists to members. Follows-up with members and providers to ensure changes are being made to meet treatment goals and to determine which recommended changes to drug therapy were implemented. Assists Manager or Director in the collection and tabulation of data for reporting purposes. Participates in the daily workload of the department, performing Pharmacist and other duties if/as needed. Interviews job applicants and provides hiring recommendations to Manager or Director. Provides staff training and coaching; helps identify and provide for training needs, in collaboration with the Manager or Director. Keeps Manager or Director apprised of operational issues, including staffing resources, program and system needs. Develops policies and procedures as needed.QualificationsJob QualificationsRequired Education Doctor of Pharmacy from an accredited pharmacy school. Continuing education required to maintain an active pharmacist license.Required Experience 3 - 5 years experience as a Clinical Pharmacist. ASHP-accredited residency or 2-3 years post graduate experience. Current knowledge and expertise in clinical pharmacology and disease management. Clinical skills related to medication therapy management.
