Licensed Social Worker needed at Skilled Nursing Facility

Ivy at Great Falls provides a range of services, including skilled nursing care, post-acute medical and rehabilitation care, hospice care, and rehabilitation therapy. Ivy at Great Falls is looking for a Social Worker to join our team! Position Highlights Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers. Establish relationships and maintain contact and referral flow with community based agencies/services QUALIFICATIONS: Bachelor's degree from accredited school of Social Work or related field is required.Must possess any certifications/licensures as required by State of employment to practice in long-term care.One to three years of supervised social work experience in a health care setting, working directly with individuals, is preferred.Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse is preferred.
