JOB SUMMARY

The incumbent in this position is responsible for coordinating the care and services of rheumatology patient populations across the continuum of care for outpatient services. Promotes effective utilization, monitors healthcare resources, and assumes a leadership role with the multi-disciplinary team to achieve optimal clinical, financial, and resource outcomes through daily operations and oversight of the practice.

ESSENTIAL JOB FUNCTIONS

  • Supports and practices behaviors consistent with the mission and philosophy of Sheridan Memorial Hospital and department/service.
  • Coordinates the daily care of rheumatology patients in conjunction with the physician. Collects, reviews, and analyzes information for evaluating patient care outcomes. Consideration is given to the appropriateness of care based on the age of the patient.
  • Facilitates and ensures that comprehensive, multi-disciplinary collaboration of care occurs with the patient, family, physicians, and other healthcare providers. Coordinates the plan with the patient, family, multi-disciplinary team, external healthcare providers, and insurers.
  • Assesses, coordinates, and facilitates patient’s progression throughout the continuum of care. Works with the physician and multi-disciplinary team to establish agreed upon care plans.
  • Coordinates the development, implementation, and evaluation of patient-focused outcomes. Performs ongoing evaluation of patient-focused outcomes, analyzes variances, and implements strategies to resolve them. Utilizes performance improvement principles to assess and improve quality.
  • Assists in successful implementation of programs to improve coding and reimbursement for rheumatology services including general knowledge of the revenue cycle, coordinating the implementation of the electronic medical record, understanding and managing appropriate charge entry system, and medical records management. Assists with completion of deficiency reports in a timely manner.
  • Assists in identifying referring physicians and assuring to the extent possible that strong relationships are developed and preserved.
  • Assists the physicians in addressing clinic related issues, assists in problem resolution within the practice, and serves as a liaison and representative for the clinic. Participates in trainings and serves on committees as deemed appropriate by SMH and the Physician Manager(s).
  • Collaborates with other healthcare providers in developing, implementing, and evaluating educational materials for patients and their families.
  • Identifies needs and sets goals for own growth and development; meets all mandatory organizational and departmental requirements.
  • Maintains competency in all organizational, departmental and outside agency safety standards relevant to job performance.
  • Supervises activities of nursing students, and support staff. Plans and organizes activities in nursing services and clinic operations to ensure patient needs are met in accordance with instructions of physician and hospital administrative procedures. Coordinates activities with other patient care units, referring providers, and hospital departments.
  • Organizes orientation and in-service training for nursing and office staff to ensure compliance with annual educational, safety, and regulatory requirements consistent with hospital policy and procedures.
  • Assists in formulating budget, in collaboration with SMH and Physician Manager(s), and practices due diligences in developing clinic operations within the annual budget guidelines. Processes all accounts payable information, purchasing, credit card reconciliation in collaboration with Physician Manager(s).
  • Responsible for supply inventory, staff scheduling and training, and payroll practices. Collaborates with SMH Administration and Physician Manager(s) during hiring and termination process. Monitors staff performance, facilitates effective teamwork, and submits annual staff performance review in conjunction with physician manager to the SMH HR department.
  • Ability to effectively coordinate care in a complex practice model with a highly interactive multi-disciplinary team of healthcare professionals. Will be faced with competing demands of available resources while maintaining patient-centered care. Work will include setting common goals, merging resources, providing education, and cross training roles.

Supervisory duties and responsibilities include:

  • Writes and conducts annual performance appraisals and ensures regular ongoing feedback, coaching, and communication with staff.
  • Ensures high level of performance standards and achievement to meet organization needs and business goals.
  • Provides development opportunities for staff to broaden and enhance skills and abilities.
  • Administers organization policies and procedures.
  • Ensures compliance with safety policies and good housekeeping.
  • Manages initial training of new employees and ongoing training as required.
  • Promotes an environment that fosters teamwork and commitment to satisfy customer requirements.
  • Participates in Safety meetings, including reporting on daily safety brief as requested.
  • Attends leadership meetings as requested.

POSITION QUALIFICATIONS

Education / Experience / License and Certifications

  • Current Wyoming license as a Registered Nurse without restrictions.
  • BLS certification, required.
  • ACLS certification, preferred.
  • Two years of supervisory experience, preferred.
