Active collaboration with nursing and physicians in monitoring and evaluation of CORE Measure compliance, quality of care and outcomes related to evidence based standards of care as well as hospital protocols/procedures of patient care and safety

Demonstrate ability to lead and facilitate a PI team as opportunities for improvement are identified, in collaboration with staff and management

Support the management team and staff in utilizing PI tools, such as PDCA. Assist in maintaining PI documentation in patient care areas

Make recommendations to changes in policies, procedures and clinical guidelines to improve clinical outcomes based on recommendations from clinical service areas

Collaborate with nursing and physicians to collect and utilize clinical criteria related evidence based guidelines

Conduct surveillance, chart reviews, documentation audits, and education activities for patient safety and performance improvement

Serve as a clinical resource for continuous performance improvement in clinical initiatives, such as TJC, CMS, NQF, State QIO and others as identified
