This position covers the Salt Lake Valley, On Call every other week. Weeknights from 5:00 PM to 8:30 AM and weekends from Friday at 5:00 PM to Monday morning at 8:30 AM. Base pay for answering after hours messages is $450 a week. Visits are paid separately.

Responsibilities include:

Take patient / family phone calls and do telephone triage - Current average of 1-2 calls per weeknight and less than 6 total calls over the weekend.

Make visits as appropriate for things like: troubleshooting catheter issues, re-bandaging a wound, assessing a patient after a fall.

Admit of new patients after hours - Current average of 1 admit per weekend, some weekends there are none but there could be several, depending on referrals and patient needs.

Make scheduled visits that must be done after hours. These visits include daily or every other day wound care. - Current average of 2-3 scheduled visits over the weekend. Weeknight scheduled on call visits aren't common but may happen if we admit IV patients that need visits in the evenings.

Hospice post mortem visits as needed - Our hospice census fluctuates between 10-15 patients.

Requirements include:

Licensed as a Registered Nurse by the State of Utah

Strong organizational skills

Attention to detail.

Ability to build rapport with patients

Strong problem solving and critical thinking skills

Ability to communicate effectively, both verbally and in writing

Current Utah driver's license and automobile insurance

CPR certification

Hospice and home health experience is preferred.
