HSRC's Redwood Coast PACE program is seeking applicants to work in collaboration with the clinic team and provide primary care for participants.

The Redwood Coast Program for All-Inclusive Care for the Elderly is looking for a full-time primary care physician to join our team. The PACE program is a national model of care, and we are the first rural program in California, opening in September of 2014. We have two sites, and our program continues to grow.

The position is Monday through Friday 8:30-5, with on call responsibilities to back-up nurses and nurse practitioners on first call, with occasional first call responsibilities. The model is high intensity, low productivity care. We average 4-8 visits per day, in clinic, home visits, RCFE visits, SNF visits and occasionally in hospital visits.

To be in PACE our participants are 55 and older, and complex medically as well as psychosocially in many cases. We have an interdisciplinary team approach, with social work, nursing, dietitian, recreational therapist, physical and occupational therapists, transportation, and home care.

Applicants for this position should have comfort with high acuity medical issues, preferably with some inpatient training and experience. They should also be comfortable with end-of-life care, palliative care, and mental health care. Teamwork and communication is essential. We interact often with specialist and hospitalist colleagues, adult protective services, and participant families. One must be self-directed and comfortable with a variable schedule, as we must respond to the acute needs of our participants as well as to their chronic primary care needs. One must also have strong computer skills. This position may require splitting time between our Eureka and Fortuna site. The work at PACE is deeply satisfying and in many ways what most of us pictured when we decided to go into medicine. We look forward to speaking with interested candidates.

Qualified applicants have a medical degree, and at least two years of experience caring for seniors.
