Rehabilitation Specialist (Baltimore City)

We are seeking a Rehabilitation Specialist to become a part of our team! You will be a key contributor within our interdisciplinary team and incorporate knowledge of each client's conditions and needs to ensure appropriate, individualized and quality care delivery.

Responsibilities:

Assess client’s individual diagnostic, social and family needs in order to develop treatment plans; and write progress notes and discharge summaries.

Construct necessary recommendations for treatment modification or discharge.

Develop and maintain clinical records to ensure compliance with COMAR regulations.

Along with the Program Director, oversee rehabilitation coordinators by monitoring care and progress in the Psychiatric Rehabilitation Program.

​Qualifications:

Master's Level licensed mental health professional (LGSW, LGPC, LMSW, CPRP, LCSW-C, LCPC)

Minimum of 2 years direct care experience working with emotionally disturbed youth

Strong organizational skills

Excellent written and verbal communication skills
