WISe Team Coordinator (Wenatchee)

Coordinates and facilitates the WISe process for youth and families to include advanced care coordination, facilitation of meetings and ensuring the phases and activities of WISe are completed for each client served by CHSW in the WISe program. Has access to protected healthcare information (PHI) or confidential identifiable information (CII), both paper and electronic, as necessary, to perform related job duties and responsibilities.


Prepares for Child and Family Team (CFT) meetings to include: developing meeting agenda with assistance of appropriate CFT member(s), scheduling meeting and inviting all CFT members, preparing visual aids or tools to be used to facilitate meeting process

Facilitates the CFT meeting, guide team process, and be a central point of communication for team and clients served

Contacts CFT members who are unable to attend a meeting, and gather their input prior to the scheduled meeting date

Establishes and sustains and effective team culture and engage all CFT members and identify their needs for meeting agency mandates

Increases “natural supports” in CFT membership and the you/family integration into their community by getting to know the family history, culture, and resources and by helping the youth and family to identify and engage potential supports

Ensures all CFT members receive an updated copy of the CSCP, documentation of progress, CFT meeting activities, discussions and task assignments as required by WISe.

Works with Youth and/or Family Support Specialist to identify family support, peer support or other “system” and community resources that can assist the youth in exercising their voice during the CFT process, if needed.

Informs the youth and family of their rights (including Due Process) and obtaining all necessary consents and releases of information

Advocates for the values of a diverse population within CHSW and the Community

Performs other duties as assigned by the supervisor


Knowledge of family support principles and practice, early childhood development and family development

Proficient in MS Office

Demonstrated successful community organizing or ethnic community outreach experience

Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community


This job is performed in a professional office environment. The following list is representative of the work environment employees encounter while performing the essential functions of this job:

Typical office lighting and temperatures

Moderate interruptions

Ability to work in a confined area

Ability to sit at a computer for an extended period

Limited travel required


While performing the essential duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer and telephone keyboard; occasionally to reach, stoop and kneel

Specific vision abilities required by this job include close vision requirements due to computer work

Regular, predictable attendance is required

Three to five years of direct service experience with children and families of targeted population; three years of community/educational outreach experience; Or, a combination of education and experience that demonstrates the required knowledge, skills, and abilities required

Agency Affiliated Counselor Credential required after hire

Must be available to be on call 24/7 (21 and older)

A valid driver’s license is required

CPR, First Aid Certifications, Mantoux TB test record (within first 30 days)


BA degree in education, social work or related field

SALARY:$40,000 to $42,000/annually DOE
