Great work you’ll do here:This position is responsible for the planning, coordinating and delivery of quality skilled services. The Clinical Supervisor is responsible for the orientation and continuous supervision of all direct care staff and assist with supervision and coordinating all support services. In addition, the Clinical Supervisor will also fill the role of the Administrator should the Administrator be unavailable for the surveyor/auditor.
Direct responsibility for delivery of quality care that meets client specific needs and in compliance with program/payer rules and physician orders
Responsible for efficient and effective utilization management in accordance with client needs, physician orders and payer requirements
Facilitates communication and coordination of care between clients, families, caregivers, physicians and other service providers as necessary
Participate in on call rotation
Proficiency in home care Conditions of Participation, coverage and qualifying criteria for Medicare, Medicaid and insurance plans
Assures efficient coordination of caregivers through case conferences and scheduling process
Assures current and accurate clinical records in compliance with documentation requirements and program specific rules for coverage and qualifying criteria
Assures clinical records are maintained in their original state and not improperly altered or fabricated
Plans and assures the adherence to budgetary guidelines
Exercises appropriate expense control management
May perform OASIS SOC/ROC/Recertification visits and regular skilled nursing visits as required
Gathers patient intake information from referral sources and completes necessary tasks to admit patients appropriate for home health services
Directs and supervises clinical team to review documentation for necessity of services, quality care and compliance with physician orders and the plan of care to ensure quality care and reimbursement
Works with DOPS or Administrator to coordinate internal compliance review and monitoring activities including monthly, quarterly and annual audits as required
Facilitates the audit process for clinical and billing purposes to improve the efficient delivery of client care and to assure appropriate reimbursement for services rendered
Oversees personnel management functions of direct reports including staffing, performance reviews and disciplinary actions
Provides a plan for staff retention and development
Assures appropriate training to meet agency and regulatory requirements
Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines

Current Registered Nurse license in the state of practice
Three (3) years of clinical experience is required. Two (2) years of home care experience with one (1) year of management (supervisory) experience in a clinical setting
Current CPR certification
Valid driver’s license and automobile insurance
