Community Care Coordinator

General Description

The Community Care Coordinator is responsible for loving the vulnerable and at-risk in our community, offering different types of support and services provided by Jubilee REACH and also through warm referrals to partner community organizations, including churches.

Working directly with families in crisis, the Community Care Coordinator gains a deep understanding of the needs and walks with the clients to ensure they feel loved and cared for.

Specialized Skill Sets:

  • Experience and demonstrated success in serving a diverse community with deep physical, emotional, and spiritual needs

  • Social and emotional intelligence and the desire to serve humbly and compassionately

  • Strong verbal and written skills with the ability to deliver clear messages and listen effectively in Spanish and English

  • Able to take initiative

  • A demonstrated ability to work with colleagues and volunteers while creating an atmosphere of cooperation and motivation

  • Able to work with deadlines and pressures while maintaining attention to detail, diligence and commitment to excellence

  • Strong administration and planning skills

  • Excellent follow-through and attention to detail

  • Ability to learn and use web-based platforms to manage client-information and services provided

Main Duties and Responsibilities

  • Love people in our community who are referred to Jubilee REACH or come to us for support in these areas:

    • Food

    • Financial Assistance

    • Clothing

    • Housewares

    • Furniture

    • Learning English as a second language

    • Dental Needs

    • Legal Support

    • Christmas Gifts for Children

  • Maintain and deepen relationships with our community partners, such as the Bellevue School District, City of Bellevue, Non-profit partners and Church partners to collaboratively provide support for clients.

  • Refer clients to partners for services Jubilee REACH does not provide

  • Refer clients to Church partners when requested

  • Follow up with clients to ensure needs are met and check-in on needs as prompted


  • Must be 21 years or older

  • Pass and State (WSP) background check, National background check, and reference check

  • Minimum HS diploma or GED equivalent

  • A heart to serve and to love our neighbor

  • Minimum of 2 years of experience of volunteering and serving your community

  • Bilingual in Spanish and English is a must
