​ SCHEDULE (This is a non-exempt/hourly position) Monday-Friday 8:00 am - 4:00 pm, with flexibility to meet consumer and/or program needs. Some nontraditional hours (evening, weekends, and holidays) may be required, on-call rotation may be required. ABOUT THE ROLEThe Team Lead works closely with the Supervisor - Community Programs to maximize productivity, compliance, quality, financial performance and individuals served in our homeless services programs. Responsible for leading and understanding harm reduction; housing first model; essential resource allocation of services; best practices in homeless services and maintaining continuity of care of individuals served. The Team Lead is also responsible for their own caseload of individuals served to provide housing and case management services towards the housing first model. Implements PMHS policies and procedures timely and effectively and monitors compliance of such. Maintains departmental policies and goals per DHS and HUD guidelines. These functions are performed in accordance with all applicable laws and regulations and Pittsburgh Mercy’s philosophy, policies, procedures and standards. RESPONSIBILITIES Uses a housing first, strength-based, and trauma-informed approach to housing goal planning. This includes holding clients accountable for guidelines and expectations outlined in the handbook.

Completes resident housing goal plan by assessing the client’s needs within first seven days of intake.

Completes intake and exit assessments and data entry into HMIS for each client within the 72 hours, when applicable

Assists the clients with referrals for permanent housing, employment placement, and non-cash benefits (Medical/ SNAP)

Provide direct care services to individuals served.
