More importantly, it empowers you to enact change and deliver the results that continuously improve our ability to provide our community with care.TheNurse Navigator (NN) Neuroscienceserves as an advocate and coach for the stroke patient ensuring they receive appropriate support and medical care during hospitalization with preparation for discharge or transfer. The NN will perform follow up phone calls complete with documentation and program specific reports. The NN will collaborate with the multidisciplinary team to identify stroke patients with high risk health practices and develop self-management and wellness plans to assist in improving health risk and behaviors. The nurse will address patient needs throughout the care continuum to reduce gaps in the care process and provide individual and group education, wellness activities, health assessments, and screenings. This is a full time or part time position which may require flexibility of hours and is eligible for overtime pay.Experience/Training:At least 5 years RN experience required. Bilingual in English & Spanish is strongly preferred. Stroke disease management and/or care coordination background strongly preferred.Education:Bachelor of Science in Nursing or higher required. Diploma or transcripts on highest degree earned required.Licenses/Certifications:Current active unrestricted license issued by State of California.Current American Heart Association BLS provider required. CNRN or SCRN Certification or related, preferred.Skills:Motivational interviewing skills strongly recommended, excellent rapport building skills with patients and providers, understanding of community resources, an ability to “meet people where they are” and a capacity to serve as a member of the team. Strong working knowledge of evidence-based practice. Patient-centered focused, detail oriented and flexible.Must be able to work independently and with a team.Able to project a positive attitude and be self motivated. Exemplary communication, coordination and problem solving skills.
