This is a housing crisis resolution system model that works in conjunction with coordinated entry. Its purpose is to leverage the resources and resiliency of the literally homeless family and to divert them from ever entering the traditional homeless shelter and transitional housing system and to rapidly rehouse them.

1. Utilize compassionate yet practical problem solving conversations to identify the family’s strengths and resources

2. Listen for information that will help understand how to effectively tailor services for each family’s critical needs

3. Work with families, once housed, for up to 30 days to develop plan to quickly move to safe, stable, longer term housing

4. Utilize innovative methods that do not rely on traditional bio-psycho-social assessments to evaluate housing readiness

5. Work with the family to identify options for doubling up, re-establishing lease terms with recent landlord, or find other permanent housing options. This may include direct advocacy with the landlord or family to mediate or resolve issues that would prevent the option.

6. Utilize HMIS and other required reporting systems to enter data that will evaluate whether the project is successful in diverting homeless families from the homeless housing system. Households returning to homelessness are not considered or counted as a failure.

7. Ensure eligibility of families (which can include those paying more than 50% of their income toward rent and utilities, sharing housing with friends, family or a roommate, and/or who are in a stable and safe but temporary arrangement or who are literally homeless or living in domestic violence housing)

8. Perform initial assessments that must include health and safety risks that may indicate diversion is not a good option. Families are prioritized in compliance with the Consolidated Homeless Grant program requirements.

9. Ensure that funds are used correctly for coaching, case management, utility payments, arrears and deposits; moving costs assistance such as moving vehicle rental, storage costs; legal fees required to access and maintain housing; interpreter/translation fees; car repairs, gas vouchers, bus tickets related to child care, employment, education, or fees related to employment. This fund cannot be used for direct rental payments.

10. Attend all peer learning calls with grant manager and other grantees as part of the cohort learning model

11. Produce the narrative response to required questions every six months

12. Track and record client level data

13. Attend regular program staffing meetings with the program manager

Preferred:

1. Experience in providing direct client services.

2. Experience in working with individuals and/or families in crisis.

3. Current Washington Driver’s License with an acceptable driving record and proof of automobile insurance, or access to reliable transportation

4. Ability to translate documents English to Spanish for publication
