Community-Minded Enterprises is recruiting for a

HUB Pathways Community Health Worker

Community-Minded Enterprises (CME) has been serving the Spokane community since 1999. Our vision is to provide integrated services that build resiliency in individuals and families. This position supports the overall mission and vision of Community-Minded Enterprises & the Disability and Access Services Initiative. The HUB Pathways program provides transitional services to people recently released from the Spokane County Jail system.

The Pathways HUB Community Health Worker (CHW) will engage with project participants and complete a comprehensive assessment of their needs/issues including, but not limited to: health, social, behavioral health and economic. Once needs are identified, the CHW will work with the participant to address those issues. The CHW serves as a link between the reentrant and existing community resources. Through home visits the CHW will assist the program participants to overcome barriers to health, social services, education and employment by performing the below duties.

Primary Duties & Responsibilities:

 Provide care coordination services of up to an average case load of 35 unduplicated participants

 Interview participants and conduct comprehensive assessment of the participants needs including, but not limited to; health (physical & behavioral health); health insurance; medication management; family & relationship issues; housing; finances; employment; education; transportation; & recreation/leisure

 Assures that client understands the process and completes all necessary paperwork

 After initial assessment, meet with social service professionals to determine the degree of need and formulation of a case plan for the participant

 Review participants care plan developed by service providers and assist in appropriate follow-up activities

 Enter all relevant demographics, provider services, and accessed resources into coordinating data system.

 Conducts outreach and of the program to referral sources and community partners

Qualifications:

 Associates Degree in Addiction Studies or similar field and minimum 5 years of experience working in the field of substance use disorders and/or mental health.

Additional desired qualifications: WA Certified Peer Counselor preferred.

 Successful Completion of the CHW curriculum or ability to acquire within 60 days of employment required

 Previous experience with and training in Motivational Interviewing & Trauma Informed Care preferred

 Clear understanding of the eligibility criteria of needed community/social service resources

 A demonstrated comfort level working with people & situations that can feel frustrating, unclear, & seemingly hopeless; Ability to work with a positive attitude in stressful situations.

 Demonstrated commitment to cultural relevancy, diversity & inclusion.

 Ability to work independently, self-motivates, prioritizes work & manages time effectively.

 Excellent written & verbal communications skills & proven ability to act in a professional manner.

 Flexibility, enthusiasm, organization, & a positive attitude.

 Computer literate & proficient using all Microsoft Office programs and QuickBooks experience.

 Must have current, valid driver’s license, insurance & access to reliable transportation & able to pass a criminal history background check.

The Pathways HUB CHW is a benefited full-time position with funding guaranteed thru December 31, 2020 but with the potential for extension.
