OVERVIEW

The Guide & Activities Coordinator embraces the philosophy of long-term care culture change while coaching the self-managed work teams of Shahbazim (C.N.A.’s) in a manner that is respectful and collaborative. They also serve as a specific resource on enriching the social, psychosocial, and therapeutic needs of Elders. The Guide & Activities Coordinator coaches, directs, and monitors the quality of care and quality of life of the elders in the Green House; working with Elders and Shahbazim to provide opportunities for Elders to enjoy the highest possible quality of life through meaningful activity. The Guide & Activities Coordinator is also responsible for overseeing compliance, competence, and operational issues and to provide monitoring and assistance to individual Shahbazim and the team, directly or through the team coordinators. The Guide & Activities Coordinator acts as the direct supervisor to the self-managed work teams of Shahbazim and is responsible for evaluating the outcomes of their work. They report directly to the GHLS Administrator

QUALIFICATIONS· Preferred experience as a CNA or willing to obtain CNA certification.

· Activities Professional Certification (or ability to become certified) required.

· Two years of experience in a social, recreation, or relevant program development and/or oversight preferred.

· Degree in human resources, social work, or a related field beneficial but not required.

· Familiarity with or the desire to learn Medicare/Medicaid rules and regulations.

· Experience in guiding and supervising large, self-directed teams or a strong desire to learn.

· Must be creative and innovative in the development of activities that maintain or improve physical and cognitive abilities and enjoy working with the Elders.

· Must be available to work alternative schedules. Hours are full-time day shift with some evening and weekend assignments. On-call availability to answer questions or assist with problem-solving required.

· Basic office and business computer skills.

SKILLS· Must be able to stand, squat, kneel, push, and pull.

· Must be able to lift 30 lbs.

· Must be able to interact with customers and co-workers in a friendly and professional manner.

· Organized and efficient with good attendance is a must.

· Must have a current driver’s license, be able to push wheelchairs with Elders, move equipment, and assist Elders with offsite functions.

· May be required to perform other tasks as needed and requested by employer.

TRAINING· Facilitate ongoing training and complete the Green House Guide training program.

· Complete Green House Implementation Manual training.

· Complete Shahbazim training.

RESPONSIBILITIES· Continuously coaches and nurtures the self-managed work team’s interpersonal interactions.

· Assesses education, information, training, coaching, and feedback needs of the Self-Managed Work Teams

· Provides information and tools that assist stakeholders (elders and self-managed work team) in their decision-making process.

· Observe and coach the relationship between Shahbazim team and the Clinical Support Team (CST).

· Review and coach the self-managed work team’s decision-making process and provide examples of effective approaches to decision making.

· Monitor overall regulatory compliance.

· Work with HR regarding all hire/dismiss decisions (the Guide may choose to seek input from the teams, but it is never the role of the team to assume these responsibilities.)

· Interact with all staff in a manner that is respectful and collaborative.

· Sits on various committees within the organization which are responsible for developing policies and procedures to assure quality of care and quality of life.

· Serves as a resource, consultant, and mentor for the Shahbazim and facilitates individual growth in all staff.

TASKS

· Reviews the self-managed schedules of the Shahbazim with the Shahbaz Scheduling Coordinator to assure compliance with staffing requirements, regulations, and the organization’s policies.

· Visits each Green House daily to monitor, coach, supervise, and correct as well as to evaluate individual levels of engagement and staff completion of activities.

· Attends team meetings to provide support and guidance.

· Discusses observations/concerns about the care of the elders with the Shahbaz Care Coordinator of each Green House Home and Director of Nursing. Place issues for team discussion on the team meeting agenda and seek feedback.

· Discusses observations/concerns about housekeeping issues with the Shahbaz Housekeeping Coordinator. Place issues for team discussion on the team meeting agenda and seek feedback.

· Discuss observations/concerns about scheduling issues with the Shahbaz Scheduling Coordinator. Place issues for team discussion on the team meeting agenda and seek feedback.

· Discuss observations/concerns about dietary issues with Shahbaz Food Coordinator. Place issues for team discussion on the team meeting agenda and seek feedback.

· Obtains a regular verbal report from each Coordinator in the house.

· Completes an in-depth assessment after admission of each Elder and provides specific materials identified through the assessment process or other means to the Elders for meaningful activity pursuits.

· Completes all MDSs, charts quarterly progress notes, and attends care plan meetings.

· Coordinates the establishment of an Elder Council, as well as writing and posting Elder Council minutes, and the Elder Newsletter.

· Regularly reviews activities charting for accuracy, completion, and to serve as a source of information for the program

· Assures appropriate and meaningful group and individual activities are available to Elders in GHLS home by observing activities regularly (daily, in so far as possible)

· Plans, organizes, promotes, and evaluates the program and activities.

· Creates individual activity opportunities for all newly admitted Elders as per GHLS Activities policy.

· Performs group or individual activities on both a scheduled and spontaneous basis, thus setting an example and building relationships with Elders.

Job Type: Full-time

Pay: $20.00 - $24.00 per hour
