Job Description
Assists the primary nurse/therapist in the provision of supportive care and comfort measures to hospice clients as directed by the personal care service care plan. Requires current CHHA certification.
1. Provides personal care services including bathing and other hygiene activities which enhance the client’s physical comfort and well-being.2. Performs household services (i.e. meal preparation and cleaning) as directed by the primary nurse through the plan of care.3. Assist/provide ROM as indicated in hospice POC.4. a . Takes and records vital signs as directed.b. Recognizes and reports changes in the client’s physical, mental and social status to the primary nurse and/or Clinical Director of Hospice.5. Documents care and services provided during home visits in timely manner per policy and procedure.6. Electronically documents in timely manner.7. Attends and participates in case conferences providing information regarding the client’s status and response to care and services received.8. Participates in agency activities.a. Attends agency staff meetings, IDG.b. Attends a minimum of 12 hours of in-services per year.
Qualifications
Requires current CHHA certification.
