Community Registered Nurse Per Diem

Conduct assessments and aide supervisions in patients' homes

Assess patient head to toe including medications & diagnoses and document

Supervise home health aide/personal care aide in all duties

Create Plan of Care for aide to follow and orient to POC

Active NYS RN license required

Home care field experience preferred

Good assessment and documentskills
