The County of Sonoma Department of Health Services seeks candidates for Senior Public Health Nurse positions in Adult Protective Services (APS), and the Home and Community Based Alternatives (HCBA) Waiver Program.

The APS Senior Public Health Nurse (PHN) works both independently and in close participation with the APS Social Work team and Mental Health liaisons. The primary responsibility of the Senior PHN is to assist County Social Workers with investigation of reports of abuse and neglect/self-neglect, in particular those areas pertaining to the health and safety of elders and dependent adults, while working in a fast paced environment. The Senior PHN is available for home and facility visits, phone consults, and other consultations when a nursing medical opinion is needed or a recommendation for services indicated to protect the client. The Senior PHN helps to identify immediate and ongoing risk factors and protective issues related to the health, safety, capacity, and ability to protect self while identifying possible resources to reduce risk in order to support the client in the least restrictive environment and strengthen client capacity for self-maintenance. The Senior PHN performs any combination of comprehensive nursing assessment, intervention, and coordination of care through home or institutional visits in addition to providing consultation and education to multidisciplinary teams and community partners, The Senior PHN will not carry a caseload but instead provide a collaborative and consultative role to the County Social Workers.

Home and Community Based Alternatives (HCBA) Waiver Program

The HCBA Senior PHN plays a lead role in the local administration of the state-wide HCBA waiver care management program. The HCBA Senior PHN provides comprehensive case management services to medically fragile individuals in the community and is responsible for conducting home visits addressing physical, cognitive, functional, psycho-social, nutritional, medical, nursing, and rehab needs. The Senior PHN works independently utilizing nursing assessment and problem solving skills relevant to provide case management services, as well as recommendations and interventions to promote the overall health, safety, and support of the HCBA client at home. The Senior PHN helps to identify immediate and ongoing risk factors while identifying possible resources to reduce risk, in order to support the client in their home. The HCBA Senior PHN provides advocacy and education to community partners and the multidisciplinary team of care providers regarding ongoing health care needs.

The ideal candidates for these positions will possess:Experience working in community settings with elders, dependent adults, and/or medically fragile individuals

Excellent verbal and written communication skills

Working knowledge of modern public health practices and community based resources

Strong planning and organizational skills

Experience conducting home visits to vulnerable populations in the community
