Job Description

We are looking for an RN to join our team of professionals. The hours of this position will be 8 am to 5 pm, Monday - Friday and no weekends or holidays.

The Case Manager’s primary responsibility is to educate patients about their chronic disease through support and oversight.

The Case Manager will work well with the clinical team. This person will need to be able to maintain library of health promotion and patient education materials and resources; provide on-going supportive and/or case management functions in accordance with the problems, needs, and strategies identified within the service plan in order to help the patients to achieve the stated goals and objectives; maintain close communication with the consulting provider, nursing staff, and behavioral health consultant for input regarding medication compliance, side effects of medication, and medication changes, and alerts members of the team of any changes in patients which may require a need for more aggressive intervention; responsible for developing plan of care, service coordination, and linkages to key community services; completes TB reporting, including medication management for patients who test positive and required public health reporting; patient nurse visits, including TB and chronic disease education; triage by phone or walk-ins, as needed.

During the Covid-19 pandemic; the Chronic Disease Case Manager's primary responsibility will be triaging patients.

Qualifications

The qualification for this position include RN in the state of Nebraska, BLS, 3 years nursing experience, computer skills, self-starter and ability to work with minimal supervision.
