Now hiring a Project Director for a non-profit organization (Ellensburg)

The Care Coordination Project Director will be part of a small team responsible for driving

the work of the Kittitas County Health Network forward. This position is 100% funded and dedicated to a federal Rural Health Care Coordination Grant which seeks to implement a community-based care coordination

system in collaboration with community partners in Kittitas County. The project’s focus is on

improving access to health and social services for people who frequently utilize emergency

systems, individuals and families who are in crisis, or individuals and families who are at risk.

The position is responsible for managing the grant award at a strategic level as well as day to

day grant oversight and management, financial and programmatic reporting, developing

subcontracts with partner agencies, managing the care coordination consortium, and

overseeing the implementation, development, evaluation, and expansion, of the community
