Position Overview:The Case Management Assistant acts independently under the direction of the RN Case Manager or Clinical Social Worker to prioritize and manage multiple demands in an ever-changing environment. The Case Management Assistant participates in the coordination and implementation of discharge plans for the patient through critical thinking, teamwork, and communication. The Case Management Assistant applies discharge knowledge and skills to coordinate and implement the integrated discharge plan developed by the RN Case Manager and/or Clinical Social Worker. This position implements processes of efficiently and effectively coordinating movement through the continuum of care to ensure timely discharge. The Case Management Assistant must maintain effective communication between the case management team, care providers, patients, families and external vendors. The position is also responsible to document all discharge planning activities appropriately in the medical record.Qualifications:

High School diploma or equivalent education / experience.

Nine (9) months experience in a direct patient care setting OR health education in an acute care hospital, skilled nursing facility, home health/hospice setting or medical office setting.

Basic knowledge of discharge planning/care transitions processes in an acute care facility, skilled nursing facility, home health/hospice or medical office setting.

Working knowledge of medical terminology AND must complete a medical terminology course within 12 months of hire ( one (1) year recent Case Management experience may be substituted for the medical terminology course ).

Must be able to travel in accordance with any requirements of your job (if MMC and/or own vehicle used on MMC business, must possess valid California Driver's License and upon request proof of liability insurance).

Occasional PM or weekend shift, as department specifies.

PREFERRED QUALIFICATIONS

AA Degree in a business/health related field

Experience in Case Management support role.

Experience with word processing and data management including Midas Care Management, EPIC and other MS Office Suite products.

SKILLS & KNOWLEDGE

Basic knowledge of discharge planning/care transitions processes in an acute care facility, skilled nursing facility, home health/hospice or medical office setting.

Ability to type 25 wpm with accuracy.

Proficient in using a computer to accurately enter and extract data, send and receive email, calendar appointments, and use task lists as will be using a variety of computer software programs.

Ability to read, write, hear, and verbally communicate in English to the degree required to perform the job.

Demonstrated ability to efficiently manage own time with minimal supervision.

Willingness to act and dress in a professional manner at all times.
