looking for 2-3 days per week of coverage and some weekends

will cover shifts on 1st and 2nd shift

inpatient and clinical pharmacy setting

Position Summary:Responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapyWhat the Clinical/Inpatient Pharmacist will need:

Doctor of Pharmacy

Pharmacist license and controlled substance license issued by the Michigan Board of Pharmacy. Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager)

ASHP PGY-1 residency, or two years of related experience

Flexible/on-call scheduling may be required

What the Clinical/Inpatient Pharmacist will do:

Provides drug therapy management and assessment.

Identifies and resolves potential and actual drug related problems.

Assists physicians in designing dosing regimens based on patient-specific dosing parameters.

Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.

Provides support to the Pharmacy & Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug therapy, etc.

Participates in performance improvement initiatives and research as appropriate.

Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant.

Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.

Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.
