    Duties

    Summary

    The Facility's RN Transfer Coordinator (TC) coordinates all referrals to and from the inpatient units. SVAHCS averages 50 transfers to and from the facility. This does not take into account the referrals to our facility that are not accepted. The TC works with other facilities within and out of VISN 19. RN Transfer Coordinator provides coverage as the Traveling Veteran Coordinator and is the back up for the Home Health Nurse Coordinator. This position monitors all community care emergencies.

    Responsibilities

    • Assure patient safety is paramount in all care provided
    • Collaborate with VA as well as non-VA staff to assure quality and cost-effective patient care is provided
    • Works in collaboration with UM Coordinator in appropriateness of admissions. Assure nursing practice is congruent with professional and regulatory standards
    • Serves as the physician surrogate in the transfer process of patients to other VA facilities or non-VA as needed from the assessment of the clinical case mix status of the patient
    • Coordinates both the exchange of clinical information about the patient and the logistics of moving the patient between facilities. This includes consideration of special mode travel when necessary
    • Notify appropriate personnel of the pending admission, including date of admission, mode of transport, accepting physician and area the patient is to be admitted
    • If care needs cannot be accommodated at this facility, provide options to the caller for healthcare facilities that may be able to accommodate the patient needs
    • Screen admission calls and obtain pertinent clinical information about the requested admission, determining if the needed care is able to be provided at this facility, and requesting medical records for review
    • Delivers fundamental knowledge-based care to assigned patients while developing technical competencies.
    • Completes initial assessments and reassessments of assigned patients as directed

    • Completes plan of care for patients and updates plan of care based on assessments

    • Administers medications in a safe and efficient manner following established policies

    • Inserts and manages peripheral intravenous access
    • Manages central venous access sites, infusion of continuous fluids and administers IV push medications under direct supervision of preceptor
    • Completes admission and discharge of patients/residents in accordance with established unit policies

In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
