St. Joseph Home Care Network is looking for a Hospice RN Case Manager for their Memorial Hospice location.
Location : Santa Rosa, California
Schedule : Full time - 32 weekly hours
Shift : 8 Hours - Week Days
Job Summary :
Manages an assigned patient / family caseload from admission through the patient’s death. Develops and updates the patient / family Plan of Care working collaboratively with the members of Interdisciplinary Team (IDT).
Identifies patient / family care needs, implementing nursing interventions as appropriate. Evaluates the results of interventions and adjusts the Plan of Care accordingly.
Demonstrates knowledge of costs related to patient care services and manages according to department guidelines. Communicates changes in patient / family condition to the Patient Care Coordinator and team members in a timely manner.
Understands the details of insurance reimbursement for nursing services.
Skills :
Good verbal and written communication skills Demonstrates a sound understanding of nursing practice with dying patients.
Self-directed with ability to work collaboratively and manage complex and multiple situations.
Minimum Position Qualifications :
Education : Registered Nurse
Experience :
2 years experience as a RN
