The AIM Clinical Program Support Specialist (CPSS) supports the Advanced Illness Management (AIM) Program with clinical and non-clinical administrative and operational program functions such as:

Tracking AIM referrals

Processing patient enrollments

Running operational or system level reports to support key program functions and data quality management requirements

Interacting with referral sources and local AIM staff to ensure timely and effective care coordination

Scheduling patient visits

Reconciling AIM patient census

Supporting coordination of patient care among segments of the AIM program

Transcribing clinical notes between electronic health records

Performing chart audits

The AIM CPSS works closely with AIM PSS, branch staff and leadership, HH intake and scheduling teams, referral sources, and Sutter Health affiliates to ensure service excellence to all internal and external customers of the AIM Program

The AIM CPSS reports to the AIM Program Support SupervisorQualifications:Education:Highschool diploma required. AA in nursing preferred. Current valid California LVN licensure or higher. Current BCLS/CPR certification is required. Valid California driver's license with proof of auto insurance.Experience:

Required:

Minimum 1 year experience working in a healthcare environment

Experience using electronic health records; navigating and documentation

Preferred:

2 years experience working in a healthcare environment

Experience working in Home Health/Hospice/Palliative Care

Knowledge:

Required

Basic knowledge of medical terminology

Skills:

Required

Strong interpersonal and customer service skills

Strong problem solving abilities

Strong commitment to teamwork and customer service both internally and externally

Proven data entry and general computer skills; Email, WORD, Excel

Ability to handle sensitive issues, conflict with or among others

Ability to effectively plan, organize and prioritize work

Ability to maintain a positive and professional image
