Job SummaryUnder the supervision of the Program Manager of the Homeless Service Center (HSC), the Case Manager I will be part of a team that is responsible for providing outreach and supportive services to individuals experiencing homelessness in Venice.

ESSENTIAL DUTIES & RESPONSIBILITIESThe Case Manager’s key areas of responsibility include:

Key Areas of Responsibility & Essential Duties:

  • Outreach to clients in order to build trust and rapport with individuals experiencing homelessness.
  • Assess homeless individuals utilizing the Vulnerability Index (VI) SPDAT.
  • Link clients to physical health, mental health and other supportive services.
  • Develop housing plans with client link clients to appropriate permanent housing programs.
  • Assist clients to enroll in mainstream benefits and obtain identification (e.g., California ID, birth certificate, social security card).
  • Document services for each client and enter appropriate data into the region’s Homeless Management Information System (HMIS)

Qualifications

QUALIFICATIONS

Knowledge, Skills & Abilities:

  • Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent administrative and organizational skills.
  • Strong knowledge of homeless services is preferred.
  • Bi-lingual Spanish/English is desirable.

Experience:

  • Two years’ experience providing services to highly vulnerable populations is preferred.

Education:

  • Bachelor’s degree from an accredited college or university in social work, human services or a related field.

Direct Reports:

  • This position has no direct supervisory responsibilities.

Computer Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.

Certificates, Licenses and Registrations:

  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Valid automobile liability insurance.

Travel Requirements:

  • To and from the work site and other travel as needed.

Status: Full-Time/Non-Exempt

Salary: DOE
