Job Summary

The Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care while adhering to the mission of St. Joseph Center.The Case Managerprovides office and field based mental health and representative payee services to adults and/or older adults diagnosed with severe and persistent mental illness and often, co-occurring substance abuse disorders.

ESSENTIAL DUTIES & RESPONSIBILITIES

The Case Manager I’s key areas of responsibility include:

Key Areas of Responsibility:

  • Provide therapeutic interventions to develop life skills, including, but not limited to money management, budgeting, accessing community resources, maintaining residence, and other skills as needed
  • Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation, and prevent relapse risks

Essential Duties:

  • Provide advocacy to deter evictions and linkage to ay needed services, such as medical/dental, mental health, substance abuse, life skills training, self-help, meaningful community activities, volunteer services, job placement, etc.
  • Implement case management plan goals emphasizing coordinated strategies for addressing mental health/substance abuse issues, relapse preventions, medication management, and any other risk factors that could impede permanent housing.
  • Provide ongoing assessment of client progress in attaining goal plans
  • Utilize a harm reduction model when working with clients
  • Provide complete documentation of all plans, disbursements and other case activities.
  • Maintain agency set productivity per DMH contract
  • Work closely with SJC clinicians in order to determine consumers’ needs.
  • Provide field visits as necessary.

Qualifications

QUALIFICATIONS

Knowledge, Skills & Abilities:

  • Must be highly motivated and a self-starter.The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.Must have excellent administrative and organizational skills.
  • Knowledge of homeless services and resources.Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • A bilingual background is a plus.
  • The position requires an ability to work flexible hours including some early mornings and evenings.

Experience:

  • One to two years’ experience providing case management services to low-income individuals in a community setting with homeless or at risk population.

Education:

  • Bachelor degree from an accredited college or university in social work, human services or a related field.

Direct Reports:

  • This position has no direct supervisory responsibilities.

Computer Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.

Certificates, Licenses and Registrations:

  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Valid automobile liability insurance.

Additional Information

Status: Full-Time/Non-Exempt

Salary: DOE
