Job Description

$5,000 sign-on bonus after the completion of an introductory period! The Transfer Center at Community Health Systems is seeking experienced Licensed Clinical Social Workers to work night and day schedules within our triage telehealth team.

This LCSW will be responsible for consultative behavioral health assessments in a telehealth setting, including the initial response, psychiatric triage assessment, designation of appropriate level of care / disposition, and initiation of placement / referral services.

Our LCSW team works collaboratively to improve the flow of Behavioral Health patients from ER and inpatient settings into Behavioral Health programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

This LCSW position is responsible for responding to requests for consultative behavioral health assessments by phone and telehealth including the initial response, psychiatric triage assessment, designation of appropriate level of care / disposition, and initiation of placement / referral services.

The position works collaboratively to improve the flow of behavioral health patients from ER and inpatient settings into appropriate programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

Examples of the work our Licensed Clinical Social Workers do day-to-day;

Ensure every patient interaction and assessment promotes the well-being and safety of the behavioral health patient.

Make psychiatric risk assessments of behavioral health patients to assist making appropriate level of care decisions.

Provide assistance with a patient's discharge safety plan, in conjunction with generating the best patient clinical outcome, as well as formulating diagnostic impressions based upon review of systems, history, symptoms, and current presentation.

About this team

The Behavioral Health Transfer Center at Community Health Systems is a state of the art clinical assessment and patient placement hub.

Composed of Registered Nurses (RN), Social Workers, Therapists, and Providers, this team works collaboratively to provide behavioral health assessments and referral / placement services for patients located throughout CHS hospital system and affiliated providers who may benefit from inpatient and outpatient behavioral health services.

Qualified applicants will have strong clinical training in behavioral health and need exceptional clinical and problem solving skills to be successful in this position.

Additionally, applicants must be able to show proven dedication to quality patient care experience. Having the ability to work within a fast-paced patient care environment helps, too! The ideal candidate will also have the ability to utilize various means of technology, using multiple systems at one time to ensure the needs of the patients are met accordingly.

Requirements you need

Licensed in the state of Tennessee without restrictions (LCSW, LPC / MHSP, LMSW )with the ability to obtain licensure in additional states as required.

Nurse Practitioners must have a Master’s Degree in Nursing or a Doctorate in Nursing from an accredited college or university.
