JOB SUMMARY:

Under general direction, is responsible for entry level professionalcommunity health work within the new SA Kids BREATHE (Building Relationships, Effective Asthma Teaching in Home Environments) Program. Individual in this position will provide asthma education and case management services to children and their families in their homes, and work with the child's school nurse and physician. Working conditions are primarily in a community-based environment and an office environment. Exercises nosupervision.The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Employees in high-risk positions will be requiredto obtain relevant immunizations, per SAMHD Policy DM13.2. Review and consideration will be given to requests for exemption from this policy.The San Antonio Metro Health District (SAMHD) is committed to advancing health equity throughout the community and to creating a work environment in which all staff feel valued, respected, and accepted. Work Hours: 8:00 a.m. - 4:30 p.m. Monday - Friday; flexible hours in early evenings and weekends to meet with working families, as is routinely needed; will workfrom home some days

Qualifications

  • High School Diploma or GED equivalent (recognized by the TexasEducation Agency or a regional accrediting agency).
  • Two (2) years of experience working as a Community Health Worker(CHW), health educator, community outreach worker, or communityorganizer.
  • This position requires a valid Class "C" Texas Driver's License OR a valid driver's license from another state with the ability to obtain a valid Class "C" Texas Driver's License within 30 days of becoming a Texas resident. Use of your own personal vehicle may at times be required and proof of Liability Insurance will be required.
  • Any applicant selected for this position must have a background checkcompleted with results satisfactory to the City of San Antonio andsuccessfully complete the San Antonio's Cash-Handling CertificationTraining.

PREFERRED QUALIFICATIONS:

  • Bilingual in English and Spanish.
  • Community Health Worker Certification.
  • Experience working with vulnerable populations.
  • Experience working with individuals with asthma and their families.
  • Experience working with a team to provide case management for a health concern.
  • Experience helping people understand a health condition and develop strategies to improve their health.
  • Experience working with children and families.
  • Ability to work flexible hours, includingroutine early evenings or weekends to meet withworking families.
