Responsible for responding to inbound inquiries for disease management or medication alert programs.

Serves as a subject matter expert.

Primary duties may include, but are not limited to: Provides information to members eligible for disease management or medication alert programs.

Makes outbound attempts to program members to provide health information according to program standards.

Receives or places calls to members who have failed to enroll with disease management to encourage their enrollment in the correct disease.

Conducts behavioral and needs assessments to identify and address individual member knowledge, skills, and behavioral, social, financial needs, and other needs.

Coordinates specific health coaching and support to address needs identified by the member to address objectives.

Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.

Coordinates with other health professionals, provider/s, and external resources to provide feedback on member goal attainment and address clinical, SDOH, and other needs.

Monitors and evaluates the interventions and modifies based on feedback from member and provider/s.

Trouble-shoots patient difficulties, which include medications, appointments, services, finances, living and family situations.

Compiles and maintains a list of resources to close care gaps and address concerns.

Qualifications - ExternalHealth Educator

Requires BA/BS in appropriate field of specialization; 2 years of related experience in direct care health education exercise instruction or patient education; certification or advanced certification and/or license appropriate to field of specialty as required; or any combination of education and experience, which would provide an equivalent background.

Prior experience in health coaching disease management in a call center environment and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.

Qualifications Health Educator

Requires BA/BS in appropriate field of specialization; 2 years of related experience in direct care health education exercise instruction or patient education; certification or advanced certification and/or license appropriate to field of specialty as required; or any combination of education and experience, which would provide an equivalent background.

Prior experience in health coaching disease management in a call center environment and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.

Health Educator Sr

Requires BA/BS in appropriate field of specialization (Health Education, Social Work, Employee Benefit Analyst); 4 years of related experience in direct care, Health Education, Exercise instruction or Patient Education; certification or advanced certification and/or licensure appropriate to field of specialty required; or any combination of education and experience, which would provide an equivalent background.

Prior experience in health coaching, disease management in a call center environment and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.

MS preferred.

Preferred:

Understanding of disease management.

Master's degree in health education, social work, or other health or behavioral health related field strongly preferred.

Related experience in health education, employee benefits, or patient education; or any combination of education and experience, which would provide an equivalent background preferred.

National Board Certified Health and Wellness Coach (NBHWC), Certified Health Education Specialist (CHES or MCHES), LCSW, and/or certification preferred.
