Position Title: Medical Case Manager-LPN (Psychiatric preferred)

FLSA Status: Part Time Non-Exempt

Required Travel: Daily Local

Salary: Salary Range $22.00 - $24.00/hr

Position Summary

The medical case manager is responsible for medical case coordination of all individuals receiving services, ensuring that individuals are receiving appropriate medical care.

The Medical Case Manager will work with case managers and caregivers to provide information regarding medical concerns, medication administration training, and interpretation of health care services.

Essential Duties and Responsibilities

1. Establish rapport and a support system for clients, Family Care Givers, Host Home Providers and Case Managers in order provide safe and optimal services.

2. Collaborate with Case Managers on medical issues, attend medical appointments when requested, provide vital signs as needed, and provide lab reports as requested.

3. Attend home or hospital visits as needed for medical consultation.

4. Teach and/or oversee all QMAP medication administration training.

5. Teach and/or observe all specialized medical trainings.

6. Develop and review medical and preventative protocols and review Professional Medical Information Pages (PMIPs).

7. Audit medical records and ensure compliance with doctor orders and recommendations. Build and maintain the computerized Medication Administration Record Therap Program.

8. Administer Abnormal Involuntary Movement Scale (AIMS) screening and Nurse Health Assessments.

9. Coordinate all medication issues with the Pharmacy, fill Medication Reminder Boxes and/or oversee designee and dispose of discontinued and expired medication and ensure requirements with Schedule II medications are met.

10. Attend team meetings, case coordinate in conjunction with program managers/directors, and provide medical information and training to staff as requested.

11. Performs administrative activities such as maintaining files, processing paperwork, and filing.

12. Meet with individuals receiving services on health-related issues.

Education Requirements

Required:

Minimum of a Licensed Practical Nurse and two years nursing experience, preferably with the client communities we serve or Psychiatric care.

Preferred:

Minimum of 3-5 years I/DD or DD experience.

Registered Nurse with 3-5 years I/DD or DD experience.

Certifications and Licenses

Required:

1. Valid Regular Driver’s License, with acceptable driving record.

Medical CPR, 1st Aid, CPI, SIP
